South Riverdale Community Health Centre (SRCHC) is a non-profit, multiservice centre that provides primary health care, social and community outreach services with an emphasis on health promotion to residents of Riverdale and South East Toronto.
Client Care- Coordinator
FULL TIME POSITION
The Client Care-Coordinator will work across three organizational teams (Newcomers & Families, Urban Health and Chronic Disease Prevention and Management). As a member of a multidisciplinary team, the Client Care-Coordinator will:
Responsibilities:
· Act as a frontline community contact person and assist clients with complex needs in navigating the services available within SRCHC and the external health and social services and settlement sectors.
· Outreach to newcomer communities
· Provide centralized intake support to new SRCHC clients with complex needs.
· Provide case management supports, crisis supports; and employ coordination, navigation and community bridging to improve access to health, social, legal and other services for individuals and their families.
· Provide cross organizational on-call crisis response on a rotating basis.
· Assist in locating required community resources by liaising with community groups and other health/social service agencies to provide referral information for clients, and to make referrals to other community health centre professionals.
· Act as a care coordinator by ensuring all available and required resources are in place and are accessed appropriately.
· Provide relevant short term counselling services as appropriate.
· Facilitate cultural interpretation for clients’ appointments.
· Facilitate transports and/or accompany clients to medical appointments, court, community programs and other appointments.
· Advocate for clients on an individual basis and address system issues.
· Maintain up-to-date client files as specified by the recording procedure.
· Participate as a member in all team functions e.g. program planning, team meetings, case conferences, etc…
Qualifications:
· Degree/diploma in Social Work/Social Services work or equivalent education and experience.
· Certification or eligibility for membership in Ontario College of Social Workers or Social Service Workers.
· Three to five years experience preferably in a community health setting.
· Experience working with newcomer men, women and youth
· Knowledge and experience of harm reduction practice
· Ability to communicate in a second language
· Solid advocacy skills and knowledge of community programs, health, social, legal and other service systems in Toronto as well as an awareness of and sensitivity to the health issues of a diverse community.
· Excellent assessment, written and verbal communication skills, counseling and interpersonal skills.
· The ability to use creative strategies that address systemic barriers, increase access to community resources and improve the quality of life of clients.
· Demonstrated ability to work effectively both independently and as a member of an interdisciplinary team.
· Understanding of the social determinants of health and the ability to apply a critical analysis in their work.
· Understands the integrated Community Health Centre Values.
· Computer literate.
