South West Community Care Access Centre
The NP is a self-directed practitioner who provides holistic patient care based on advanced nursing and basic medical management knowledge and skills for a selected patient population. The responsibilities will include but will not be restricted to providing palliative care for clients in their own homes as they near end of life, liaising with clients, families and all health care providers and using clinical assessment, monitoring and management skills to provide the best possible client care. The NP will be a key member of the Client Services Team, helping clients to manage pain and symptoms and avoid unnecessary hospitalization. The successful candidates will report jointly to the Program Director, Emergency Services & Base Hospital, LHSC and the Client Services Manager, South West CCAC. These positions are located across the South West region in Elgin, Oxford, London-Middlesex, Huron-Perth and Grey-Bruce.
